Your full name and address and the address and office of the person who caused the notice or levy to be issued to you. If you have a bank account or other financial account, tell the court that during the case you deposited funds, or received payments from the creditor. If you have not filed an Answer of Answer yet, or do not have a bank account or other financial account to deposit funds, you may file your request in the federal district court for the District of Columbia.  If the person who caused a notice or levy is not in court, the order gives you the right to seek collection by sending a Demand Letter to the person who received the notice or levy.
